Are Product Visions Happening at All Levels?

I wanted to ask that because it often seems to come hand in hand. There's sometimes confusion between what is the mission that we're on and then how do we get there? I think it's helpful to kind of understand at least in this context what the distinction is. So my next question would be who sets them and at what level? It's interesting because I've had my own journey of coming to understand this over time as I've evolved my own career from being an individual contributor to being a product leader.
