Defensive decision-making means that you do not follow what's best for your company, but you want to protect yourself in the first place. It's very similar to situation in medicine, study of the study shows that about more than 90% of American doctors say they practice defensive medicine. So these are factors which I would put in the center rather than inner workings like defensive decision-making and a culture around that's a negative error culture.
