Intravenous (IV) iron administration has shown no significant increase in severe adverse events or infections among a large patient cohort, indicating its general safety. However, caution must be exercised in patients with active infections, particularly bacteremia, due to the preference of bacteria for iron, warranting measures to restrict iron during inflammation. Misinterpretation of infection risks related to IV iron can lead to undue hesitance in treatment for patients who are not actively infected, as the risks in such cases remain minimal. In practice, it may be strategic to delay IV iron treatment until later in the hospitalization unless clear signs of infection are present.
