Acetaminophen level and treatment

To assess if a patient needs treatment for acetaminophen overdose, draw an acetaminophen level after 4 hours and use the Matthew Rheumach nomogram. If above the treatment line, give acetylcysteine. For patients who took it with opioids or anticholinergics or delayed release, draw levels at 8 and 12 hours. If positive, treat immediately. The nomogram is great but only works for narrow cases and must be used within 24 hours of exposure.
