Complexity and Treatment Challenges in Psychotic Depression
Psychotic depression presents complexity in treatment due to the lack of immediate improvement compared to major depressive disorder. In treating inpatient depression with psychosis, it takes longer and often does not respond to antidepressants alone. The response rate to antidepressants in psychotic depression is low, around 20 to 30 percent, but when combined with antipsychotics, response rates improve significantly. Antidepressants alone are not very effective in treating psychotic depression. Treatment options mostly involve a combination of antidepressants and antipsychotics, or in severe cases, electroconvulsive therapy (ECT). Other treatments like transcranial magnetic stimulation have shown less effectiveness in these patients.
