There has been a lack of accountability for fake news stories in the past, such as the case of weapons of mass destruction in Iraq. Despite the clear misinformation and the devastating consequences of such stories, the business took years to admit the mistake and mostly blamed it on one individual. The lack of reflection and accountability within news organizations for major errors like this is concerning and there should be a deep sense of regret for such significant mistakes.
