Considerations when using Sotalol and Dofetilide
When using Sotalol and Dofetilide, healthcare providers should monitor for potential QT prolongation, especially in patients already on other QT prolonging medications. It's important to check electrolyte levels, particularly magnesium, and be cautious in patients with slower heart rates due to the increased risk of torsades with Sotalol. Dofetilide is known to increase the risk of torsades, leading some EP groups to avoid it if possible. Both medications are cleared renally, making them less suitable for patients with progressive chronic kidney disease. In cases of missed doses, patients may need to be reloaded in the hospital to prevent complications.
