As men age, an increase in PSA levels in the blood is common due to prostate enlargement, along with a higher percentage of free PSA. A low or declining percent free PSA along with rising PSA levels could indicate health concerns, although not necessarily cancer. Starting PSA screening at age 45 is recommended to establish a baseline, as prostate cancer is rare in this age group. Men should proactively monitor their PSA annually without solely relying on their doctors. An increase of more than half a point per year or surpassing certain thresholds (2.5 or 4) could signal a need for further evaluation. A high percent free PSA indicates lower worry for health issues.
