If you have direct inhibitors and no reversals, use PCC for everything. Patients with coagulopathic deficiencies need fresh frozen plasma. Remember that calcium is part of the clotting cascade. For massive resuscitation, add back calcium. Use cryoprecipitate or fiber engine concentrate for fiber antigen levels less than two. If platelets are less than 50, replace them. Consider using DDAVP for patients with renal failure and euremic platelets. If on anticoagulant, use PCC.
