The Relationship Between Testosterone Replacement Therapy and Prostate Cancer Risk

In patients with symptomatic hypogonadism, testosterone replacement therapy (TRT) is generally recommended despite concerns regarding prostate cancer risk. Key factors influencing the decision to proceed with TRT include PSA levels, family history, prostate size, and benign prostatic hyperplasia (BPH). However, in the absence of prostate cancer, there are few scenarios where TRT would be discouraged, as the benefits to cardiovascular health, bone health, muscle mass, and cognitive function are paramount. For individuals diagnosed with low-grade prostate cancer, maintaining a eugonadal state through TRT is preferable, as it contributes positively to overall health while monitoring the progression of cancer. Maintaining eugonadism is considered vital, particularly when the cancer is low-grade and under surveillance.
