Dosing in the Outpatient Setting?
In the outpatient setting, the dose of hydrocortisone depends on a person's body surface area. The standard replacement doses are 7.5-10 milligrams per meter squared. For an average US male, this would be around 15-20 milligrams per day. The dose is usually split between morning and evening. Adjustments may be made based on symptoms and discussions with the endocrinologist. Prophylaxis is not necessary for physiologic doses, but doses above 20 milligrams of prednisone can suppress the immune system and require prophylaxis for infections and osteoporosis.
