Approach to Malignant Hypertension

The first priority is to lower the blood pressure to prevent organ damage, ideally not reducing by more than 25% and maintaining it above 160 in the initial hours of treatment. Auto-regulation in such patients is impaired, causing a linear drop in flow with decreasing blood pressure, emphasizing the need for caution in lowering blood pressure rapidly.
