S2 Ep99: When Perimenopausal BLEEDING is the Problem

Years before estrogen production completely shuts down, hormone levels start to fluctuate wildly during that special time of life known as perimenopause. Perimenopausal hormonal fluctuations result in the same symptoms that many women experience when they are in full menopause, hot flashes, vaginal dryness, and insomnia. But there is a bonus symptom that post-menopausal women don’t experience - irregular and unpredictable bleeding.  In this episode, I discuss: WHY perimenopause women have irregular bleeding How your clinician should evaluate abnormal bleeding Strategies to manage perimenopausal bleeding  Progestogen  Birth Control Pills  Progestogen IUD  Endometrial Ablation  Hysterectomy  Do nothing!  What to do if there is HYPERPLASIA For more information on this topic:  Episode 23:  Welcome to Peri-menopause Episode 40 Don’t Go with This Flow- Post Menopause Bleeding Episode 56 Too Young For Menopause? with Dr. Hannah Short Episode 57 Uterine Cancer: Reducing Your Risk Episode 80 When Progesterone is a Problem  Hot Flash Hell: A Gynecologist's Guide to Turning Down the Heat                                And, it’s not too late to register for                               Mastering Menopause – Get Away!                  Fabulous Retreats with Real Experts to Get Real Solutions https://www.drstreicher.com/mastering-menopause-get-away Meet the Experts https://www.drstreicher.com/mastering-menopause-get-away#meet-the-experts Lauren Streicher, MD is a clinical professor of obstetrics and gynecology at Northwestern University’s Feinberg School of Medicine, and the founding medical director of the Northwestern Medicine Center for Sexual Medicine and Menopause. S2 Ep94: HORMONES and the VOICE

Have you ever wondered why so many women professional singers retire in their prime, but the guys keep on going?  I googled “musicians over the age of 70 who are still performing” and Paul McCartney, Elton John, Marvin Gaye,  Mick Jagger, and dozens of others popped up. The list was really long for guys, and really short for women.  One reason is that when estrogen levels take a plunge, the change in hormones has a significant impact on vocal frequency and range. In this episode, my guest is Dr. Cheryl Kinney, an expert on the impact of hormones on the voice.   How estrogen and progesterone influence voice throughout life Why testosterone lowers the voice Why thousands of boys were castrated in the 1700s How voice changes during a menstrual cycle Why some birth control pills alter voice What the cervix and throat have in common  The impact of menopause on the voice The impact on professional vocalists and voice-over actors. How and why obesity influences hormonal impact on voice Medical conditions and specific medications that alter voice How to find a specialist who treats hormonal vocal changes New research showing that voice changes can be a biomarker for hormonal changes and other health conditions About Dr. Kinney www.drcherylkinney.com Dr. Cheryl Kinney is a gynecologist in in Dallas, Texas where she is Director of the Center for Female Health and Hormone Disorders.  Dr. Kinney is on the Board of Trustees of The Menopause Society and is on the Editorial Board of the journal Menopause. Dr. Kinney also serves on committees for the International Society for the Study of Vulvovaginal Disease and the International Society for the Study of Women’s Sexual Health. In addition to her expertise regarding menopause on health and sickness in the novels of Jane Austen and other 18th and 19th-century British authors.  Lauren Streicher, MD is a clinical professor of obstetrics and gynecology at Northwestern University’s Feinberg School of Medicine, and the founding medical director of the Northwestern Medicine Center for Sexual Medicine and Menopause.                           Uterine? Ovarian? Cervical? Lung? Breast? Colon? Hormone therapy really needs a PR firm. Most people think it is right up there with cigarettes and asbestos as something that will more likely than not, give you not only breast cancer, but also other life-threatening cancers. In this episode, I am going to set the record straight and tell you the facts when it comes to using systemic estrogen (both birth control pills and menopause hormone therapy) and your risk of getting ovarian cancer, uterine cancer, cervical cancer and the three most common non-gynecologic cancers. S2 Ep90: The Connection Between ACHING JOINTS and ESTROGEN

It’s no secret that there is a connection between menopause and hot flashes, but there are many other symptoms that are consequences of a lack of estrogen beyond night sweats, vaginal dryness, and bone loss. And joint pain is definitely on that list. So, what is the impact of menopause on arthritis and joint pain? Is estrogen therapy the answer?  In this episode I am joined by Dr. Stephanie Faubion, the Director of Mayo Clinic’s Center for Women’s Health, chair of the Department of Medicine at Mayo Clinic in Jacksonville, Florida, the medical director for The Menopause Society, and the author of The New Rules of Menopause: A Mayo Clinic guide to perimenopause and beyond  We discuss: The relationship between JOINT ACHES and ESTROGEN If it is the CARTILEDGE, BONE, or BOTH that appears to be impacted by the loss of estrogen  Why so many women are diagnosed with FIBROMYALGIA at the time of menopause Why SLEEP APNEA also increases in Menopause and contributes to fatigue If HORMONE THERAPY should be prescribed to help achy joints and arthritis The increased prevalence of CARPAL TUNNEL SYNDROME and FROZEN SHOULDER If joint pain is an indication of OSTEOPOROSIS If most DOCTORS are aware of the relationship between joint aches and estrogen When joint aches are NOT RELATED to MENOPAUSE The impact of estrogen therapy and JOINT REPLACEMENT OTHER FACTORS that contribute to achy joints post-menopause and what can help The importance of ERGONOMICS and joint pain Why I do SQUATS while I brush my teeth Dr. Faubion’s New Book: The New Rules of Menopause: A Mayo Clinic guide to perimenopause and beyond  To find a Menopause Society  certified menopause practitioner Other Related Episodes: Episode 22:  Dreaming of Sleep with Dr. Faubion  Episode 46 Finding a Menopause Clinician Who Will Listen  Episode 61 Boning Up on Bones: Part 2 with Dr. Kristi Tough DeSapri (Prevention and Treatment)  Episode 67- How Long Can You Safely Take Hormone Therapy? S2 Ep89: Faking Orgasm

While not every woman fakes orgasm, and most don’t do it routinely, it’s the rare woman who hasn’t done it at least once. Early research indicated that roughly 55% of women have faked orgasm at least once in their lifetime. This general pattern has since been confirmed in more recent studies, some reporting rates as high as 74%. In this episode I am joined by certified sex therapist Rachel Zar, LMFT.  We discuss:  WHY women fake orgasm What factors predict the likelihood of faking orgasm If it’s ever OK to fake orgasm What a woman should say to her partner instead of faking How often, and why, men fake orgasm Rachel Zar LMFT, CST, is an AASECT-certified sex therapist with a master’s degree in Marital and Family Therapy from Northwestern University.  She is currently in private practice in Chicago and is completing a PhD in Clinical Sexology.
