Understanding Stress, Trauma, and Loss During Fertility Treatment with guest Dr. Loree Johnson
When seeing patients, I always recommend people to gather their Fertility TEAM, and the T is for therapist. I'm so excited to have Dr. Loree Johnson share her valuable info with all of you today. In this conversation, we're going to be talking about some of the mental health impacts of being a fertility patient. I'll be asking Dr. Johnson about the difference between Infertility Stress and Infertility Trauma, we're going to talk about grief as it relates to fertility (and how it's different than other grief), and she'll share how to navigate the stress of being pregnant after having loss. Dr. Loree Johnson is a Licensed Marriage and Family Therapist (LMFT) with more than 25 years of experience as a clinician, educator, and clinical supervisor. After her first-hand experience with fertility challenges taught her the importance of self-care for managing the physical and emotional stress that come with the fertility process, coupled with the lack of representation she saw of therapists of color in the fertility space, Dr. Loree found her purpose supporting individuals and couples on their path to parenthood. Top Reasons You're Having a Hard Time Getting Pregnant
There are many things that can impact your ability to become pregnant. In today's episode, I'm reviewing the top 10 things to consider if you are TTC. 1. Environmental Toxins. Avoid microwaving food and plastic containers or bottles. Always store food and glass or stainless steel containers. Million Marker offers a test to find out if you have in-body toxicity. 2. Chemicals in food and everyday products. Verify that the products you are using are chemical free. You can do that on https://www.ewg.org/ 3. Being sedentary. The CDC and American College of OBGYN, both recommend exercise in amounts of 150 minutes per week. Avoid over exercising, as low BMI can also cause fertility issues. 4. Food allergies or intolerances. Work with a nutritionist to find out if you have a food allergy or other food intolerance. You may also want to consider working with a specialist if you have PCOS or using Pollie.co to help you manage PCOS if you have it. 5. Blocked Fallopian Tubes. Check with your doctor to find out if this is what may be causing you to have a hard time TTC. 6. THC and Cannabinoids can affect the motility of the fallopian tube and the receptors in the lining of the uterus. This makes embryo implantation harder and it might not happen as quickly as you would want. 7. Nicotine. Nicotine can affect the DNA and the egg in the sperm cells, and it can also affect a woman's fertility such that she could go potentially into menopause earlier. 8. Blaming birth control pills. Many people think that birth control pills "cause" infertility, but the truth is that because they regulate your period they may be masking the indicators of infertility. Make sure you get your hormone levels checked with your doctor every year to confirm your levels. 9. Ignoring biology. Every woman is born with a finite number of eggs, and if you want to wait to have kids until later in life, be sure and freeze your eggs when you are in your 20s. 10. Not getting enough sleep. How to Talk to Your Doctor About Fertility if You Are Over 40
Talking about your fertility when you're over 40 is not easy, but I want women to stop apologizing for desiring a family. Times have changed. It's okay to want a baby over 40. It's not crazy, nor selfish. My approach is for all of my patients to be given their best chance at a healthy pregnancy in the shortest period of time. How do we do this? It starts by not believing everything you read. There's an international headline I saw recently on BBC news that fertility doctors are exploiting older women and "dealing false hope". Do I believe that fertility doctors deliver false hope? No way! I do not. Do I believe that it's imperative to get a diagnosis before treatment, regardless of who you are and your age? Yes! Diagnosis before treatment. Please! I want my patients over 40 to understand their follicle count. Then I want them to decide if moving forward with treatment using their own eggs is the best path forward. There are so many ways to have a baby if your own body is not making it happen there are other creative paths to explore. Egg and embryo donation and adoption are a few examples. Using the SpermQT Test to Complete Your Path to Fertility with guest Dr. Kristin Brogaard
Today, Dr. Kristin Brogaard is joining me today to talk about the Path Fertility SpermQT test. I'm so excited to have her share about the test, because as you'll hear her say "lots of infertility goes undiagnosed because men go undiagnosed, and when men go undiagnosed, women go through procedures that will not work." (Who else wants to see this on a bumper sticker?) While sperm were long considered to be just carriers of DNA, it turns out they are a lot more complex than that! Understanding the sperm side of the equation can help anyone trying to conceive better understand what's going on, and help them figure out the best plan to get pregnant. A few exciting highlights from our discussion: Male and sperm epigenetics play a big role in getting pregnant, and having a successful live birth. The SpermQT test can tell someone if their sperm is excellent, average, or poor quality. - Knowing this information can help you and your doctor determine the most efficient and appropriate next steps in your treatment or protocols. Understanding the quality of the sperm can lead to better outcomes. If it is "excellent," there is an increased likelihood of IUI working, even in cases where doctors thought sperm might be the issue with getting pregnant. If sperm quality is "poor," you may want to try making lifestyle changes to improve sperm quality, and retest. - Poor sperm quality often correlates with other longer term health issues for men, as sperm acts as a "canary in the coal mine." (an early warning system) Dr. Kristin Brogaard has a PhD from Northwestern University where she developed a novel epigenetic technology that allowed for a more accurate understanding of gene expression in health and disease cells. She is now the chief scientific officer of Path Fertility, whose goal is to raise the standard in male reproductive health, starting with male infertility. Does Sperm Really Matter?
Does sperm really matter? If you've been tuning in to all of the posts this week for "Super BALLS Week" you'll know that of course it matters! It truly does take both sperm and an egg to make a baby. Today I'll review the sperm question a bit, and explain why it matters. AND, I'll give you a lot more info about how to improve sperm quality. Here's the thing: I know that guys want to hear what they want to hear. They have selective listening. I'm not kidding when I tell you that I had someone email me recently and say "My husband's sperm tested out GREAT, and when we went over the results with the doctor we heard two different things. He thinks he heard our doctor say that it means he can just keep drinking, smoking, and enjoying marijuana and doesn't need to make any lifestyle changes. I heard something very different, which was that he should still cut back until we are pregnant. Can you help settle this for us?" And while it's an unpopular answer (at least with the guys), my response is: Men should cut back or abstain from drinking, and smoking (marijuana too) if you're TTC. Here are a few of the things I suggest to anyone bringing the sperm side of the equation to the baby making situation: - consider freezing your own sperm for future use when you are young. - get in the best shape of your life if you're trying to conceive. Sperm matters, and men's health is directly related to their sperm quality. - take some prenatal vitamins. Yes, really. Many of the same supplements that I recommend for egg quality also are great for sperm quality. - cut back on smoking, drinking, and drug use. Marijuana negatively impacts sperm. - don't cook your balls: avoid hot tubs, using your laptop on your lap, and other situations that may create high temps for the area. - find ways to cut back on stress, if you live a stressful life.
