I have unexplained infertility. How many rounds of IUI should I do before moving to IVF? (Ask The Egg Whisperer)

Recently, Brittany sent a question in to Ask The Egg Whisperer. She has a diagnosis of unexplained infertility, and wants to know how many rounds of IUI she should do before moving to IVF. Here's her question: "Hi Dr. Aimee! I am a 33 year old woman. My husband is the same age and we have been trying for about two years. I want multiple children. I had a case of PID. My IUD got infected when I was 20. The only diagnosis they have for me (I've been to two different clinics) is unexplained infertility. They say my tubes might be scarred from PID, but the saline histogram showed no abnormalities and perfect openings. Maybe endometriosis runs in my family, maybe it's PCOS. There are no definites on anything. So we've just started our first IUI with Femera, so I'm trying to stay positive. How many rounds should I do before moving on to IVF? And what can you suggest or recommend for my situation? I'm supplementing and exercising, walking with lightweights, doing yoga and trying to remain light and positive, please help." I'm glad that Brittany shared all of this background information. While she's been given this unexplained sort of diagnosis, it feels like there likely is an explanation. Whenever I have a young patient like her (and it sounds like everything else is great: No fibroids, no polyps, good sperm), the tubes in my mind are blocked until proven otherwise. She should talk to her doctor, and he may want to ask for a hysterosalpingogram or a laparoscopy, which give more information than a saline histogram. A couple of other things to consider: Since Brittany had an infection with an IUD, I worry that she could also get an infection from an IUI, which stands for intrauterine insemination. She might consider doing another test to look at her fallopian tubes. I would say because of the infection that she had from the IUD, it's possible that the fallopian tubes might be blocked and IVF is the way to go to build her family. What are some of the reasons to get no embryos after a round of IVF? (Ask the Egg Whisperer)

In this episode, a listener asks about the reasons she did not get any embryos in her latest round of IVF. She is 37, and had 25 eggs retrieved. Here's what she has to say, "What are some of the reasons to get no embryos after a round of IVF? I'm 33 years old and me to 3.7 had 25 eggs retrieved, 10 mature fertilized, but none made it to day five. I'm heartbroken and confused." I agree that this is heart breaking, and confusing. I would recommend speaking to your IVF doctor, and ask why they think why things haven't worked until now. Get all of the information you can about your fertility diagnosis. IVF can give you a lot of great information, so ask your doctor  very specifically for all your official reports. You want to know about the information from on the day of egg retrieval. Ask your doctor if there were there any physical characteristics that they saw in your eggs that would lead them to believe that this could possibly happen. Ask if there was there something about the eggs that makes them think that this could be an quality issue. And then also ask, if there something about the sperm that makes them think this could be a sperm quality issue, and lastly ask about the genetic screening that you've done and get those results. I was about to start IVF, and now my period is 10 days late. Could it be from stress? (Ask the Egg Whisperer)

In this episode, Jess writes in to Ask the Egg Whisperer why her period is late. She's getting ready to do IVF and worries that stress might be impacting her cycle.  Here's what she asks, "Hi, Dr. Aimee. I've had my baseline scan. It was fine, but my period is now 10 days late. Why? I've taken a pregnancy test and it is negative. Have I scared my period away worrying about starting IVF? How can I get my period to come? It's always so regular that I'm really frustrated. I want to get started. Please help." My short answer for Jess is that if she were my patient, I would make sure that we check baseline levels of Estradiol, Progesterone, FSH, and sometimes HCG as well. We want to see what's going on with your body, and look to see if there is a cyst. There is no need to sit at home and worry. I'm 26, and we've been trying for over two years. What should we do next? (Ask the Egg Whisperer)

Recently, Brianna wrote in to Ask The Egg Whisperer a question. On this episode, Dr. Aimee is joined by Dr. Gayane Ambartsumyan of Reproductive Partners Medical Group. So, Brianna is getting answers from two Egg Whisperers! Here's what Brianna asked, "I'm a medical student, third year, in Chicago. I am 26, and my husband is 31. We stopped using  contraception June of 2019. We were not truly trying, but hoping at the same time. I started getting worried in June of 2020 and began tracking BBT, cervical mucus, and LH strips. That's a whole lot of tracking day three! My FSH is 10, AFC is 19,  AMH is 2.94 and day 21 progesterone is 10.8. We are starting to get really discouraged. The last four months have been really tough with the time and mental energy is taking a toll on already busy lives. My husband is also a doctor. We are so excited to become parents. And aren't sure if we should explore IVF at this time? What is the best route for our hopeful family? Thank you for being so wonderful and helping educate, uplift, and empower so many women." Our short answer is that Brianna could look at ways to reduce stress. She could also consider completing the TUSHY Method set of tests: a Fallopian tube test, a sperm count,  and a genetic or carrier screen.
