Chemo recovery [16:44] Derik says: Good afternoon Brief:  My 69 year old mother has just finished her 6 month chemo 'medication' for Follicular Lymphoma. She is due her scan to assess progress, but by all other accounts seem to be responding very well.  I'm planning to put her on a mostly red meat diet to help her recover. I'm not an expert by any stretch, but have been learning a bit about paleo / carnivore way of eating and have been 90% carnivore myself for 6 month with lovely results. Weight -25kg and great bloods etc. In any case, my questions is - besides eating beef, lamb (steak + mince + liver aiming for 1.6g/lean kg mass), eggs, fish, pork, chicken, butter, cheese (weekly) and some fruit, nuts (avoid peanuts, almonds, cashew) and veg sprinkled in there - is there anything else like certain supplements that you would recommend she perhaps take generally for a while to help build up what the treatment may have destroyed.  One article explained that some B vitamins especially can be depleted after chemo. Or should the above diet be fine in replenishing her body over months? ps. I'm also keen on getting her to gently do some exercise. Any guidance or points would be very welcome. Love your work Derik 3. Can Low Carb Work for Hard Gainers? [22:16] Ben says: Hey Robb and Nicki! Long time listener, first time caller. Love the renewed mission and content - it is critical! Here's my question: Have you seen a low carb template work for people that have a hard time putting on muscle? For context, I'm 6'0", 155#, 11% BF, and 30 years old (though this problem isn't new). I split my training between CrossFit and lifting heavy compound/functional movements. I've always been a "sugar/carb burner" and since my training has been biased towards strength and CrossFit, I have always eaten high carb (i.e. 350g a day) still in the paleo/whole foods world, but still have a lot of difficulty adding muscle even at a caloric surplus. I continuously experiment with my calories and macros but have never tried low carb as I thought it would be too hard to get enough calories and it didn't make sense to me with my training. Am I missing out on gains not trying low carb? My standard labs are all in good shape - are there other tests I should be doing to see where I'm lacking (i.e. Nutritional DNA tests)? (There are obviously important factors outside of nutrition - I'm constantly trying to optimize my sleep and recovery so let's focus on nutrition for the sake of this question.)   4. Reverse Dieting [29:48] Cheryl says: Hey Robb, I've been a long-time fan of your work, have all of your books and see you largely as a voice of reason in the sea of ever-growing health and wellness opinion. So, thank you! First, a little about me for context. I'm a 63  yr old woman, 5'5" and currently weigh 244 pounds. I have hypothyroidism, and until I adopted a keto diet struggled with high blood pressure and borderline insulin resistance. My blood pressure and blood glucose both stay nice and steady now (115/70 and 89 respectively), thanks to keto. Here's my question. Back about 3 years ago I participated in a group that operated on the reverse dieting concept to restore metabolic rate, and they did it via a decent amount of protein, pretty low fat and massive carbs. My personal macros were 140g protein, 50g fat and 180g carbs. In 4 weeks, I gained 8 pounds and quickly determined that I was going in the wrong direction. After that I became a part of the ketogains community, participated in several bootcamps, and while I made some progress there, it was extremely slow, lost about 10# total, and undoubtedly gained some muscle. My slow progress with ketogains macros/bootcamp has led me to wonder once again if my metabolic rate has somehow been damaged over the years of desperate attempts to drop my excess body weight, and if so want to understand how to restore it. I've run into yet another group that espouses the reverse dieting concept, but they do it the keto way.....I've given it a go, just finished 3 weeks, and I am a little surprised at the results. My macros have been set at 60g protein, 20g carbs (total), and 145-200g fat (fat cycling throughout the week). At any average of 600-700 calories higher than any ketogains macros I had, i have somehow managed to drop 3#. Can you help me understand what's happening here? Assuming I have some metabolic damage going on, is there anything to this reverse dieting concept or what would you recommend instead? Thanks in advance for your help. Cheryl Louisville, KY 5. Estrogen in fermented soybeans vs improvements in gut health [41:19] Gage says: On a recent podcast, you discussed soy and how estrogenic it is. My question is a follow-up to that: does fermentation remove some of these estrogen-signaling isoflavones, and does it make soy less allergenic in general? I currently live in Japan, and I’m wondering whether it would detract from my mostly carnivore diet to incorporate natto in my eating regimen (for the admittedly vague goal of improving gut health). While I’m not terribly worried about my testosterone or estrogen levels, I’m still a young man and would like the most testosterone and least estrogen as I can manage. I would love to reverse this if possible, what approach would you recommend? Thank you for your time & assistance. 2. Nicotine and Wiping [13:54] Gregory says: Hi Robb and Nicki!  Thank you for having taken the time to create the AMA-platform for THRR. I was blown away with your folks' openness to receiving and responding to my exceptionally long query. I did what you had suggested; cook my veggies until they're almost baby-food, incorporate a slight caloric deficit, and keep my exercise intensity low. I still have some hiccups here and there when taking care of business in the bathroom, but I'd say that 90-95% of the time, wiping my ass is not necessary. Any time that I do enjoy a raw salad though, there's immediate change. THANK YOU. Now as for the fatty liver, CT-scans and blood tests came up inconclusive except that my liver is just slightly larger than normal, and my cholesterol is elevated (total and LDL-calc). Now, if you have time... could you help me out with looking at nicotine? I've read some of your work, as well as others purporting that it may be a "multivitamin" for the brain... having no real effect upon circadian rhythm and possibly having a neuro-protective effect against Alzheimer's, as well as aiding in treating Parkinson's disease... I recently had a conversation with someone that raised the concern of nicotine and endothelia function. I've done some perusing through literature and it appears to be a positive for angiogenesis, but a negative in that it may lead to atherosclerosis and cancer... This was looking at nicotine on its own, and via gum use-age...  Do you have any thoughts or insight? NEXT AND FINAL question... When in the evolutionary time-line of humans do you think we began wiping our asses? Our cute-cuddly fur-babies don't wipe after they pop-a-squat...I've never seen an ape do it. Like I said above, my digestion has altered in a way that I don't need to... Did cleaning the dark-portal come about after we began relying heavily upon agriculture? Not sure if this will make it to the new podcast but I really need some help from somebody in the Health Care side of things as opposed to the Sick Care side. I am an aspiring health coach so I've been fairly dialed on my nutrition for a few years now. I recently noticed a rather large bulge on my side about the size on a tennis ball which just seemed to appear out of nowhere and I tend to check myself out in the mirror when I'm in front of it without clothes on. I went in to get it checked out after it didn't subside in a few days and after an ultra sound, it was determined that I have a lipoma. This definitely isn't my first although I haven't had one in over a decade and assumed it was do to bad diet once I learned the healthy way to live but the problem is that the surgeon is saying that General Anesthesia (GE) will be necessary in this case since it involves a lot of muscle tissue surrounding it and Local doesn't seem to do the trick well enough. My question is there anything I can do to mitigate the effects of GE on the brain? I've had multiple TBIs, been under at least a few times before for different reasons and have suffered terrible health issues ranging from severe depression to memory issues reminiscent of somebody with Alzheimer's. I cycle in and out of Keto and have tried different treatments that I have been able to afford like neurofeedback and have come a long way and I'm terrified of sliding backwards again or losing even more of my memory. I'm lost on this and don't really know where else to turn but love the stuff you guys put out and wish I could retain the info like you seem to be able to do and really value your knowledge and appreciate all that you do. Any advice or thoughts would be greatly appreciated. Neuroprotective and disease-modifying effects of the ketogenic diet Nutritional ketosis delays the onset of isoflurane induced anesthesia   4. Enzymes to support gluten digestion [28:46] Chelsea says: Hi! I’m curious on your thoughts about all the enzymes out right now that target gluten digestion. I’m gluten free, but when I eat out I usually take a “glutenease” as an insurance policy in the event of cross contamination. Am I just waisting my money on these? In the past lipase enzymes really helped me when I was having gall bladder issues. Thanks!! 5. Progesterone, Cortisol, & Melatonin [32:19] Alicia says: Hi Robb & Nicki! I've been an avid follower of y'all since I was diagnosed with Celiac's disease back in 2012, The Paleo Solution was my BIBLE for a long time! About a year ago, my Chiropractor (my now boss) advised me to get my hormone's tested due to a high stress job, lack of sleep, and over training in my workouts (I was in his office at least once a week due to back and neck problems that wouldn't go away). After completing the hormone panel, they basically found out that I was running away from a bear, but I'd be dead before the bear caught me. My Progesterone was essentially non-existent, as were both my estrogen and testosterone, my cortisol was definitely higher than it should be for a 28 yr old female, and my melatonin was registering off the charts (at the time I was taking anywhere between 10-15mg a night to sleep), and my body had actually stopped creating melatonin. My question is: after cutting out Melatonin, and trying two different kinds of Progesterone supplements, realistically, can I repair the damage I've done to my hormones, and what kind of timeline am I looking at? I was a high level athlete for 10 plus years, training around 4-6 hours a day and began working an extremely stressful job with a long commute from age 26-28. I quit that job a year ago and began working at my Chiropractors office in a much less stressful position. Besides some CBD, what other things can I implement to help repair these hormones? I haven't done another hormone panel since, however, I am planning on doing another one in the next couple months. Creating a complete protein with beef protein powder [11:58] Brandon says: Dear Robb, Due to leaky gut, food sensitivities, hypothyroidism, and intense strength training in the gym, I need plenty of protein, can't get it all from my diet alone, and so need to supplement with a protein powder. However, I cannot digest egg protein, rice protein, pea protein or other vegetable protein, or whey protein in any form (concentrate, isolate, hydrolysate, etc). I can, though, digest beef protein powder. Since it does not have the correct amino acid profile like whey, however, to achieve proper muscle protein synthesis, can I supplement beef protein powder with BCAAs, or at least leucine, to create the complete protein powder I need? 2. Tendon repair? [16:33] Bill says: I’ve been active in the gym for some time now in one fashion or another. However, I’m noticing that my tendons and joints, if I’m not rea-a-al careful,  fail the “live forever” mandate.  My muscle is very cooperative and chafes at the bit for more action,  but frankly the infrastructure lags.  I’ve managed to tear shoulder tendons on both shoulder ball joints, as well as exploded a bicep tendon off the shoulder entirely. Sleep, early riser history, ibuprofen [21:47] Tim says: Hey, been a listener for I don't know how long (have a pic of me holding up the Paleo Solution September 12, 2010). I'm 52, was (still am but not by choice or need!) an early riser for 25+ years. Even though my job no longer requires that of me I still wake up around 4 every day, weekends included. I find it very very difficult to go back to sleep. I eat clean except special occasions. I do CF 5 times a week and BJJ 3 or 4. BJJ is new and I feel like it is SO HARD WHEN I WAS YOUNG THINGS WERE AUTOMATIC I didn't think about wrestling moves or positions they were just there... sorry, not relevant... but infuriating. Every once in a while I will take ibuprofen, really very rare, maybe 5-10 days a year? It seems whenever I do take it, though, I sleep until my wife's alarm wakes me up. It could of course be coincidence. Any reason ibuprofen could help me stay asleep? Any other thing I could take to mimic the effect without the drawbacks? Keto and polycystic kidney disease [10:15] Megan Hi, my name is megan, I'm a 30 year old female with an inherited disease called polycystic kidney disease. I have many large cysts covering my kidneys and liver and with an MRI scan I have an 80 percent chance of reaching kidney failure before I turn 40. There is no cure for it, my grandfather passed away from it at 47 and my mom is on dialysis waiting for a transplant. There were some studies showing previously that calorie restriction and low body weight can slow cyst growth (I'm 5'4 and 126 pounds) and Recently, the Weimbs lab has done studies in rats and cats that have shown that the Keto diet with IF is reversing the disease completely.  The pkd foundation is wanting to start human studies but it wont be for a while. My nephrologist called keto the starvation diet when I approached him about it and told me it was too restrictin, would take too long to see  he would rather me eat less meat and more grains. I have experimented with keto and overall feel better with less flank pain. So my question is, I know that animals and humans react differently to these studies, but wondering if I should do the keto diet long term anyways, instead of wait for human trials to come out, or if it may do more harm than good with my low body weight and kidney issues. See I have been on a the standard macro keto diet 70%fat 25%protein and 5%carbs for the past two years. I went keto after to my alarm going to the doctors finding my fasting blood sugar at 124mg/dL. I have experienced all the benefits of keto from normalized glucose levels throughout the day to mental clarity. My  fasting glucose is now 80mg/dL and goes into the 70s three hours post meals. My only problem is that period has completely stopped since going keto. I don’t restrict calories, only lift 4 times a week for around 45minutes and I don’t do any high intensity exercise or running. I also meditate daily and even use a red light to combat daily inflammation and stressors in life. To add to this I also am at a healthy weight for my height and have never had any weight problems all my life, I just had “metabolic syndrome” at a healthy weight. To take a deep dive I ran a ultrasound and found that I have a small cyst on my liver as well as a small amount of biliary sludge on my liver. I then proceeded to get a HIDA scan, a MRI of my abdomen, and pelvic area. Every came back normal so I then proceeded to run a hormone panel which then led me to get diagnosed with Hypothalamic Amenorrhea. I then decide that my HPA-axis most likely needs stimulation so I had a sweet potato consisting of 25grams of carbs with ground beef for lunch one day. The problem is after the meal I began having to urinate around every 15 to twenty minutes for the next two hours. I then proceeded to feel very jittery and anxious quickly following the meal. This led me to have a crash later in the day and I had to have a snack as I became profusely famished. I have never had to snack before I I eat a three large meals a day filled with whole keto foods and plenty of protein. I believe I still have insulin resistance but I want to be able to transition to a low carb style diet in order to stimulate my LH, HPA-axis, thyroid, and increase my estrogen levels in order to menstruate to protect my body from osteoporosis and other side effects of amenorrhea. I've followed your work for a while and I really appreciate how you remain so open minded and non-dogmatic. Can you please address the issue of some people developing dry eyes and mouths on low carb. I developed Sjogrens Syndrome (when I was still eating plenty of carbs) and it is doing a number on my eyes, mouth (and other mucous membranes), my digestive system and joints. Eating mostly meat is the only thing that controls the stomach pain from my longstanding IBS and more recent Sjogrens. Paul Jaminet cautions against low carb due to what happened to him on it (dry eyes and mouth) and so I am worried I will make my issues even worse if I continue carnivore diet style eating. It's hard to tell if it is causing any dryness since I am already so dry. Thanks, Kat 4. Timing of meals and BJJ training [31:35] Kris says: Hi Rob and Nicki, I'm 46 years old health conscious dad, husband and lawyer. I started BJJ about 10 months ago with my 8 year old son. I've got two stripes and am loving the training, very mental. I come from an endurance background, Ironman triathlons, marathons (Boston 3 x) and I ran 24 hours around a track last year for charity to celebrate my 45th birthday. BJJ is a whole new challenge with the mental game. My question, I eat low carb consistently and have been meat focused for the past few months with some vegetables. I intermittent fast most days for 16-18 hours typically skipping breakfast. My BJJ training is usually in the evening from 6:15 until 7:45 pm. For overall health, would you recommend having my last meal at around 4 pm before BJJ training and fasting until the next morning. Or would you recommend having a big breakfast around 11 am, a small snack before BJJ training and an evening meal after training? I love your yearly recap and am an avid listener of your podcast. I hope to one day earn my purple belt in BJJ. I am actually similar to your build, short around 5'6" but muscular with big legs. I weigh around 160 lbs with 8-10% bodyfat. Acne [35:22] Erica says: Hey Robb, I’m not sure if you’ve covered the subject of acne because I am a fairly new listener to your podcast, which I love by the way. I’ve learned so much already. But getting to the point, I am a 17 year old female that has had acne since age 10. I’ve done almost every type of treatment: oral and topical antibiotics, birth control, benzoyl peroxide, retinoids, you name it. Nothing ever seems to help, and if it does, my acne eventually comes back. I don’t want to take antibiotics or birth control because that just screws up your gut microbiome. I am a fairly clean eater (I eat paleo but I have the occasional dessert or junk food), I exercise a lot (I play volleyball and weightlift three days a week), and I get a decent amount of sleep (7-8 hours per night). My acne has thankfully become pretty mild but either way, acne isn’t NORMAL. In your opinion, what are the biggest causes of acne? I’ve heard diet, stress, dehydration, mineral deficiencies, and I’m honestly just so confused. Any advice or insight would be appreciated.
