Idiopathic Avascular Necrosis of the Proximal Femur Epiphysis
Idiopathic Avascular Necrosis, also known as Legg-Calve-Perthes disease, is explained. It involves the femoral head becoming spherical and is more common in boys aged four to eight. Earlier presentation, under the age of six, is associated with a better prognosis. The disease can manifest with symptoms such as knee pain or a limp. The stages of Perthes disease are outlined, including an irritable hip or knee phase, fragmentation phase, reosification, and eventual remodeling. The Herring lateral pillar classification, which measures the amount of height lost, is also discussed.
Slipped Capital Femoral Epiphysis (Skiffy)
Skiffy is characterized as a slip of the femoral neck epiphysis, typically occurring in obese males or adolescents with a limp. Physical examination may reveal obligate external rotation of the hip when flexed. The diagnosis can be confirmed through radiographic clues, such as Klein's line not intersecting the proximal femoral epiphysis and widened fices. Treatment options for skiffy include in situ pinning with a larger screw, consideration of open reduction or surgical hip dislocation for malunion or hardware failure, and working up for endocrine abnormalities in patients under 10 years old. Complications may include avascular necrosis, conjrolysis, and residual deformity.
