Connect Disparate Symptoms
- Celiac often presents as scattered, nonspecific complaints that only look classic when you synthesize them.
- Maintain a low threshold to suspect celiac when multiple systems (GI, skin, hair, anemia) are involved.
Use TTG IgA With Total IgA
- Start celiac screening with IgA tissue transglutaminase plus total IgA level to avoid missed diagnoses from IgA deficiency.
- Reserve broader serologic panels; add tests only if results are equivocal or inconsistent with histology.
Expect Nonclassic Presentations
- Nonclassic celiac presentations are common and include constipation and obesity.
- Don't exclude celiac just because patients lack classic diarrhoea and weight loss.
