Episode notes
MTP Initiation
- Consider hemodynamics, bleeding rate, and expected trajectory when initiating a massive transfusion.
- Make sure it's actually hemorrhagic shock before giving lots of blood products.
MTP Applicability
- Most massive transfusions occur in non-trauma situations like GI bleeds or obstetric hemorrhage.
- Current MTP protocols are largely based on trauma literature, which may not be entirely applicable.
Hemoglobin Not Reliable
- Don't rely on hemoglobin levels to trigger a massive transfusion.
- Patients can exsanguinate quickly with normal initial hemoglobin.
