What does it mean to be "born again"?

You've probably heard that you must be "born again" to enter heaven. The phrase "born again" is often used by religious folks to create another "us and them" mentality. Who is in and who is out. When religion is used to divide the world into who is included and who is excluded, we have missed the point. For Jesus, everyone is included. So, what does it mean to be "born again"?
