Error Number One, Is There a Medical History?
There was no up to date medical history. The patient had forgotten his long list of medications, and he's going by memory. He wasn't instructed to miss his a morning dose for an anti-quaglant. There were complications with bleeding that caused the patient to have his toes amputated nine months ago. His poor health combined with all those other factors meant i did not feel comfortable treating him day after day.
