The Safety First Mnemonic - What Is It?

I'd like to share another tool with you that I call the safety first mnemonic. It's hard for us to both conceptualize them in a certain note format, whether that's soap or Gerb or DAP. In this case, we're just looking at whether or not you've documented those critical safety or risk factors. And remember, make sure to document the client's presentation and your clinical response to it.
