direct clipping has a higher risk of rupture and ruptured aneurysms are associated with much poorer post-operative outcomes on average. The disadvantage of this approach is that the surgeon is effectively creating a stroke. It's not just the aneurysm that isn't filling, but it's other cerebral tissue that is downstream from theAneurysm.
