I would always favor using something like nichardipine over hydrocym which what like you mentioned is unpredictable has a long half-life and it's probably not not a good drug to use from the risk of lowering the blood pressure too much. The rare zebra ones there will be patients that you might see with who have a few chromosome cytoma crisis. Those are really best managed by with phantolamine, then as soon as the blood pressure is better controlled adding a beta blocker but only after they are well alpha blocked fortunately most of those patients are diagnosed in the outpatient setting.
