Jason: There's kind of a crisis response that really needs to happen and then it's you know you touched on learning Jason I think that's so important is if an incident has occurred. We can't prevent reoccurrences if we don't understand the drivers of behaviors and decision-making, he says. "I won't call it a silver lining because I think this just bollocks cleaning at silver lining but I think we are flawed"
