The classic approach is through a medium sternotomy, just like you would have a heart surgery through. Most people who do astronotomy to day do what's called an extended transternal timatime. That means taking everything between the frantic nerves and then the upper thimac horns that extend into the neck. But when you take every little bit of fat, you're going to have complications. The operation takes much longer. And more likely to get a d v t or something like that.
