Having a history of heart failure definitely increases the likelihood that his presentation could be consistent with an exasterbation. Even if this dishnia is explained by heart failure, we still need to explore the reason why he was tipped into heart failure. The last thing i do when walking through a defferential for any sindrum, i ask myself, is there any acute, life threatening cause of this symptom that i need to rule out emergently? So i would get an e k g a d dimer and basic chest imaging. Basic labs should also be done, and will help demonstrate if the patient has severe anemia that could explain their symptoms as well reallynt do, as usual
