The cardiopulmonary interactions of increased positive intraturassic pressure can have effects on preload to the heart as well as venous drainage from this cerebral vasculature. We need hypovolemia with isotonic fluids like normal saline to achieve normal rather than excess volume status. Using balanced crystalloids rather than saline may have beneficial effect on acid based balance, renal physiology and patient outcomes. I think clinically we should really keep SIEDH or cerebral salt wasting at the top of our differential.
