Bitfinex - What Were the First Red Flags?

Bitfinex lost billions of dollars in one of the largest hacks of all time. The exact mechanism of the hack has never been made clearr because bitfenix has never made it clear. But there's a couple of people who've had different theories about how this might have happened. It was supposed to be a two of three signing thing, wher you needed eithera bitfenes key, bitgo's key, or the user's key.
