It's not 96% of people who are symptomatic with tears, it's structural pathology. Surgeons need to be discussing themselves, do we need to be assessing fat infiltration in the muscle before we operate? There's great uncertainty as to the effect of surgery. I would always go with the patient preference. The patient is the most important person in the healthcare system. Most of the use must be respected. But if their opinion is that they feel that they should be escalated to a surgical opinion, then absolutely go and get an opinion from an orthopedic surgeon.
