Introduction

Medical errors in the United States are the third leading cause of mortality, with an estimated 250,000 deaths per year. In radiology, reported major error rates range from 2 to 6%. Prior studies identified high shift volumes to be associated with diagnostic errors. This study aims to explore such risk factors at a large tertiary care academic medical center.
