The Importance of Clinical Significance in Medicine

What we care in medicine is clinical significance. Do I make a difference? Does it matter? Is it big enough that is worthwhile? This is really what we want to answer, but unfortunately, most of the time, we are stuck with trying to answer very plain, frequentist question which boils down to statistical significance. Typically, this boils down to a p-value threshold of 0.05 for most scientific fields.
