The Importance of Tracheostomy in Anesthesia

The tracheostomy is done inferior to the cricothyroid ligament. As a general rule, we like to stay at least one ring away from the cricoid. There's probably some hybrid versions of that, although they would still be a percutaneoustracheostomy just done in the operating room.
