The current guidelines still recommend using crystalloids as a first line fluid, but they now suggest using balanced fluids like plasmilite or lactated ringers instead of normal saline. If fluids aren't enough and the patient is in septic shock, the recommended first line vasopressure is norepinephrine. The final point for monitoring and guiding resuscitation is using other adjunctive measures like capillary refill.
