There are two million people in the UK who were undiagnosed with COPD and I think that's probably reflected in the evidence base because when you look into the evidence behind why some ambulance trusts around the world are advising the use of CPAP for COPD. When you look at the evidence it's almost invariably a focus on acute respiratory failure as an entity rather than focusing in on COPD alone so I think it's a bit of an unanswered question and maybe something that we need to look at in the future. The final step which is thankfully pretty rare is that if a patient fails an IV or can't tolerate it then they might well require invasive ventilation via an ET tube but if
