What Makes a Team Agile?

If we'd had this discussion ten years ago, i would have given you a very different answer. I think one of the most common ways that scrum fails is when scrum is used to micro manage developers or testers. They're not giving a lot of leeway and how they approach their work. And then your team needs to be empowered to go make those adjustments themselves. If a team is not empowered to change, they never change, and they probably stop thinking about how they do things.
