Is Space Getting More Congested?

There are dozens of countries active in space and many commercial entities. How is it that they don't somehow become entangled with each other? There's very light regulation on what radio frequencies you can use, so that the radio signals don't interfere with one another. It's not like a nice motor way with all the cars going very fast down the same laye. Everybody's going at different heights, in different directions, at these incredible speeds. And mostly you just rely on the fact that space is big to not hit each other.
