The key to detection of an unexpected donor derived infection is suspicion on the part of the clinician. Most donor derived infections will manifest within the first month after transplantation, but some fungal and parasitic infections will occur later. Immediately alert the OPO if you suspect a donor derived infection. And always expect a question on spondyloid in the setting of recurrent gram negative bacteria.
