How to Write a Clinical Descriptor for Mental Status Exams

Psychiatrist: Don't use adjectives for the patient. Just use clinical descriptors. This is not the place to editorialize or introduce science. Every document in my term, and I say this to trainees, is a discoverable document. And so we'll go in detail into each category and some of the different descriptors that you might use.
