In sevaceous glands, about 50% of DHEA and testosterone is 5 alpha reduced to DHT. And that's why even with extremely low DHT levels, you can have intragenic acne. It's the same way with intragenic alopecia. Even though a higher dose doesn't lower the serum level, you see a bit more of an effect.
