The Death of Vladimir Komarov on the Soyuz 1

Vladimir Komarov deploys the Soyuz 1's parachute at 23,000 feet of altitude. The first parachute doesn't deployment properly and the main parachute doesn't deploy at all. His reserve parachute does release, but gets tangled up with the other parachutes rendering them both ineffective. So without any parachutes to slow them down, the Soyuz1 comes plummeting to Earth at about 89 miles per hour.
