Is There a No Blame Culture?

It is never a person's fault. It's always a process problem, he says. We can't fix the person but we're goin to fix the process. He asks why was there a dependency on a human pushingsome configuration? What didn't we build that meant that this could have been done automatically? And what was the validation that didn't happen that confirm that the change was within bounds? i think it's really importantd, i don't think it's pervasive yet.
