How to Enable Developers to Speak Up

A lot of teams and managers talk about how their meetings and retrospectives go, and it's a lot of silence. How do you stop this from happening? And if you are in a team where this is already happening, what steps can be taken to revert this behavior? Well, the dynamic you're describing is extremely common.
