The Conflict Between MD and PhD in the US

In the US, there's often a conflict for academic MD PhDs in that your institute wants you to be two people. Clinical medicine in the US is so profitable to the institutions that there's this motivation to have that. Now, being in a place with something that as far as I'm concerned as a medical is a mythical unicorn, aka a national health system of some form, where there isn't a profit motive, do you guys have that conflict or is because or for other reasons, clinical medicine being treated not quite the same market driven approach it is in the US?
