runway incursions are actally the most common kind of plane accident. Not collisions in the sky are extraordinarily rare. It's much more common to have planes bump into each other on the ground. A study done by richard haynes many years ago had commercial pilots flying in a flight simulator using a head up display, so projected their air speeds and their altitude and their pitch on to the sield in front of them. The idea is that because they're not focused on the outside world, they simply don't see it.
