When stuff goes wrong, it makes sense to try to figure out why it went wrong and what to do differently. But it's so easy to step over the line into like, stuff went wrong because engineer screwed up. And end like some one needs to suffer for this, and they will bear the burden of having done it wrong. It's really hard to learn from things that go wrong if your incentive is to protect yourself, not find out what happened.
