There are two ways to perform an exchange for infusion. The arithrocitiparess machine removes the patient's blood centrifuges it somehow like sub fractionate the r b cs, pulls out some of the sickled cells, and then you can put in fresh r bcs. You're going to need a technician and a hemotologist to run that machine. Manual exchange transfusion is a pretty simple technique, which i think is highly duable and could be, in a life savingif you have to do it.
