If I'm willing to leave a kid unparalysed and actually promote their spontaneous ventilation then there's some potential advantage to that but not in the like reduced sedation although that probably has some benefit. If you guys look at CT cross sectional CTs of adults primarily but also children with ARDS most of the disease is in the posterior parts of the lung. So supine ventilation in diseased lungs is an inherent vq mismatch right so the idea of APERV is that what if I set my eye time long enough to recruit all that crap in the back? The trans pulmonary pressure argument for APERV doesn't necessarily work because in terms of it being lung protective you're reading
