A patient was found dead in the stairwell of a VA hospital in Bedford, Massachusetts. He'd been missing since May and it's well into June here. Think about layers of accountability that mustn't exist in that hospital for that to have been even remotely possible. Incentives matter. And why do they have so little regard for their patients? Because their patients have no other options.
