What Engineers Made a Mistake?
I've been in a lot of incidents where it's the desire to be able to point and say a person made this mistake is high. I feel like personally as someone who has had to say that kind of stuff to customers a thousand times saying it was a person who did this thing. It shows so much less understanding of the event and understanding of the system than actually talking through the different components or contributing factors what are wrong.
